Can't add a new user

I just installed OpenGoo 1.2.1 on our Windows 2003 R2 server running IIS, with PHP v5.2.9 and MySQL v5.1.  Everything seemed to install fine, and I was able to create an initial "admin" user during the install.  When I login, most things seem to work, but when I click on "add user", all I get is the following.

and I can't add any new users.  Did I do something wrong when I installed it?

Try to install OpenGoo in XAMPP or WAMP first and see if you get the same error
